• An active CrowdCard account with accepted program terms is required to earn CrowdCard rewards. If you do not have a CrowdCard account, complete the form to submit your request. It can take up to 10 business days for your CrowdCard account to be provisioned.
• The CrowdCard program's point allocation is limited to activities that occurred within 30 days prior to enrollment and requires this form to be submitted to have missing rewards reviewed.
• Incentives are generally paid 14 days after opportunity qualification. If you are seeking rewards for a missing opportunity and it has been less than 14 days, please wait to submit this form.
• If you have identified a missing opportunity for rewards, it is part of the program terms that this form must be submitted within 30 days of the date that the opportunity qualified, to be considered for payment.
